Complete the process for each person whose record needs to be confirmed. If you are an adult, you must create you own account and submit a request on your own behalf. If you are creating a record on behalf of a dependent, please create your own account and then add your dependent on the screen that asks who the patient is. You can get as many records confirmed for as many dependents as needed.
